Breaking the cycle of homelessness: how Concrete’s Young Person’s Service changes lives

Lukes Story Concrete

For young people experiencing homelessness, particularly care leavers, the journey toward independence can feel overwhelming. Without a stable support network or a safe place to turn, many face complex barriers that make it difficult to move forward. At Concrete, our Young Person’s Service exists to change that story.

Our service supports young people who are experiencing homelessness, many of whom are care leavers with limited support around them. We provide not only safe accommodation, but also the practical and emotional support needed to help young people rebuild their confidence, develop life skills, and take meaningful steps toward independent living.

A tailored, person-centred approach
At the heart of our Young Person’s Service is a belief that no two journeys are the same. Our teams take a fully personalised and holistic approach, working closely with each young person to understand their individual needs, goals, and challenges.

Through one-to-one support, personalised support plans, and consistent relationship-building, our Housing Support Coaches adapt their approach to suit the person in front of them. This tailored support is crucial in helping young people overcome barriers that may have previously held them back.

Real change through trust and understanding
Luke had been living at one of our Young Person’s schemes for quite some time and was feeling stuck. Progress felt out of reach and engaging with support had been challenging in the past.

That began to change after Luke started working with his new Housing Support Coach, Pete. By taking the time to build trust and understand Luke’s individual needs, Pete was able to support him in making progress he hadn’t managed for a long time.

Luke gradually began to establish a healthier sleep routine, giving structure to his day. He made significant improvements in managing his living space, keeping it clean and organised, and started engaging with support in ways he had previously struggled to do.

He also made positive changes around personal hygiene, including washing his clothes regularly, and took important steps toward improving his wellbeing by engaging with health professionals to address long-standing health issues.

Why these steps matter
While these changes may seem small on the surface, they represent a huge step forward. These achievements highlight how tailored, compassionate support can help break down barriers that once felt overwhelming.

Luke’s progress is a testament to the importance of building strong relationships, adapting approaches, and ensuring that every young person receives support that is meaningful to them.

Supporting independence, one step at a time
At Concrete, we know that breaking the cycle of homelessness doesn’t happen overnight. It happens through patience, understanding, and a commitment to providing personalised support that genuinely makes a difference.

Our Young Person’s Service shows that with the right support, young people, regardless of their background or experiences, can begin to remove the barriers preventing them from living independently and move toward a more stable and hopeful future.
